I have the USB dongle installed as per Troy instructions.
All cable connections seem solid.
What should I do?

Try making sure your firestick isn’t fully loaded and using all its resources. Uninstall not needed apps and make sure it isn’t overheated.
In there is a setup guide too try going over this as well.

Both my FS4K’s would occasionally reboot when using an OTG cable and USB drive.
Solved the issue by adding a 5V 2amp power supply instead of using the included 1amp ps. You can find them for around $7
