Do I just download the app to my Shield and it automatically does all that stuff… or do I have to connect it somehow to the Kodi app?

Install into Kodi app and configure.
Optiklean is good for deep cleaning. Like @GraceWilson I have both installed and use EZ maintenance every day and optiklean about once every week or two.
Take a look at OptiKlean by DigiTalking (on Github). I’ve been using this plugin for around 3 months to take out the trash and optimize my databases.
